The Contract

Participants sign a contract which provides the information on the length of monitoring, the requirements to comply with the monitoring plan and the reporting requirements.  Typically contracts are for 5 years from the initial contract.

 

Monitoring

Monitoring, as outlined in the contract, determines the expectations of the participant and will document their illness management.  Participants are required to provide the documentation that shows they are compliant with their contract. Participants will be required to signed releases of information to document their progress.

 

Providers

Participants must have providers to treat the illness for which the participant is being monitored.  The type of professionals and specific treatment requirements are dependant upon the participant's illness and are specified in the contract.

 

Monitors

Participants will be assigned a monitor to report to.  The monitor will provide reports on a monthly basis.

 

Advocate Reports

Participants will be connected with advocates who   can assist with finding resources and help walk the participant through the process.  Advocates report on a quarterly basis.

 

Participant Reports

Every participant must submit monthly or quarterly reports on their progress depending on their plan. It is essential that this information is forthcoming to provide services.  Participants are responsible for all of their information.

 

Employer Reports 

The work-site monitor is a supervisor or manager to whom the participant is accountable.  The work site monitor should not be a peer, colleague, or professional partner.  The role of the work site monitor is to provide information regarding the participant's overall work performance.  Employer reports are as agreed upon

 

Board Compliance

All participants will be required to comply with all aspects of their board orders to remain in compliance with their contracts, if there is a licensing board involved.

 

Toxicology Screens

Persons being monitored for substance disorders may be asked to provide random urine screens. The participant is responsible for the costs involved in testing.

 

12 Step or Self-help Meeting Attendance

Persons being monitored for substance disorders may be asked to attending 12 step or self-help meetings and designate a sponsor. They may be required to provide written documentation of attendance at these meetings.  The number of meetings required per week will be outlined in the participant's contract.

 

Other Requirements

Each contract is individualized and there may be additional requirements based on the participants plan.

 

Discharge

If participants do not comply with the conditions of their contract, or if they choose to discontinue monitoring for any reason, a report may be filed with their licensing board or their employer according to provisions in their contract.

Program Completion

When participants successfully meet the conditions of their contracts, they will be discharged. It is our goal to assist each individual to successfully achieve the completion of their contracts.
